Annual Acknowledgment & Medication Forms (2026)
To simplify our process, we now use annual forms instead of requiring paperwork for every event.
The Acknowledgment Form covers general permissions and acknowledgments and is valid for all events throughout the 2026 calendar year.
The Medication Form provides information and authorization related to medications and is also valid for the entire 2026 year.
Please complete each form once per year—no need to resubmit for individual events.

SUNDAY NIGHT SCHEDULE
Join us in the Family Life Center for a great time of worship, games, snacks, and more. Doors will open at 5:30 pm. Pick-ups will be at The Bridge at 7:30 pm.
WEDNESDAY NIGHT SCHEDULE
Join us in The Bridge for our Youth Discipleship Class at 6:00 - 7:30 pm.
